Fyzioterapie kolenního kloubu po operaci předního zkříženého vazu / Markéta Tomsová. -- 2017. -- 86 listů, 12 nečíslovaných listů příloh : barevné ilustrace. -- Oponent Kateřina Vaníková. -- Vedoucí práce Gabriela Medunová. -- Abstract: Cíl práce: Přispět k řešení problematiky fyzioterapie kolenního kloubu po operaci předního zkříženého vazu. Metodika: Využití manuálních technik, zhodnocení stavu měkkých tkání a jizev, hodnocení rozsahu pohybu (ROM) a svalové síly v pravé dolní končetině (PDK). Posouzení chůze po rovném povrchu i do schodů a zhodnocení stability postiženého kloubu v jednotlivých fázích fyzioterapie. Na základě získaných poznatků zvolení správného postupu fyzioterapie. Výsledky: Zjištění patologií měkkých tkání a přítomnosti aktivních jizev. Snížení ROM v pravém kolenním kloubu, svalová dysbalance a nestabilita operovaného kloubu. Snížení svalové síly v PDK. Na základě zjištěných výsledků vyšetření využití vhodných terapeutických metod a konceptů: techniky měkkých tkání, postizometrická relaxace (PIR), rytmická stabilizace (RS), kineziotaping, ošetření zkrácených struktur, aktivní cvičení s overballem a therabandem, využití proprioceptivní neuromuskulární facilitace (PNF), senzomotorické stimulace (SMS) a metody dle R. Brunkow. V průběhu terapie došlo k uvolnění měkkých tkání, snížení otoku kolenního kloubu, zvětšení ROM v pravém kolenním kloubu, zvýšení svalové síly u PDK, zlepšení stability pravého kolenního kloubu, zlepšení stereotypu chůze. Závěr: Vlivem intenzivní fyzioterapie spojené s domácí autoterapií bylo dosaženo zvětšení ROM a stability v pravém kolenním kloubu, odstranění otoku, zvýšení svalové síly v celé PDK, obnovení správného stereotypu chůze po rovném povrchu i do schodů. Pacient ukončil terapii s minimálně omezeným ROM a subjektivním pocitem plné stability kolenního kloubu. Vše tedy nasvědčovalo prospěšnosti fyzioterapie a domácí autoterapie.. -- Abstract: The aim of the work: Contribution to the physiotherapy of the knee joint after the surgery of the anterior cruciate ligament. Methodology: The use of manual techniques, the assessment of soft tissues and scars, the evaluation of range of motion (ROM) and muscle strength of the right lower extremity, the assessment of walking on the flat surface and walking up stairs and the assessing the affected joint´s stability in various stages of physiotherapy. It´s based on the knowledge of choosing the correct procedure of physiotherapy. Results: The detection of soft tissue pathology and the presence of active scars, the reduction of ROM in the right knee joint, the muscle imbalance and instability of the operated joint, the reduction of muscle strength in the right lower extremity. The therapeutic methods and concepts are based on the testing results: the soft tissue techniques, the post-isometric relaxation (PIR), the rhythmic stabilization (RS), the kineziotaping, the treatment of shortened structures, the active use of overball and theraband, the proprioceptive neuromuscular facilitation (PNF), the sensorimotor stimulation (SMS) and the concept of the Brunkow method. During the therapy sessions: the releasing of soft tissues, the swelling reduction of the knee joint, the increase of ROM in the right knee, the increase of the muscle strength in the right lower extremity, the improvement of the right knee joint stability, the improvement of the walking stereotype. Conclusion: Due to the intensive physiotherapy associated with autotherapy at home the patient has achieved the enlargement of ROM and the stability in the right knee, the elimination of swelling, the increase of muscle strength in the right lower extremity, the restoration of the correct stereotype of walking on a flat surface and walking up the stairs. The patient has completed therapy with mostly full ROM and subjective sensation of full stability of the knee joint. All these conditions pointed out to the benefits of the physiotherapy and home autotherapy.
