Oponent Ondřej Kališko Abstract Cílem mé práce je přispět k řešení problematiky fyzioterapie po artroskopickém ošetření impingement syndromu ramenního kloubu. Metodika: zde je zpracována kazuistika 53 letého pacienta s diagnózou impingement syndrom po artroskopickém ošetření ruptury m.subscapularis, parciální ruptury caput longum m. biceps brachii a ruptury povrchového listu m. infraspinatus na pravé horní končetině. U pacienta jsem odebrala anamnézu, provedla jsem vstupní kineziologický rozbor (antropometrie, goniometrie, svalový test dle Jandy, statické a dynamické vyšetření pohybového systému, pohybového stereotypu dle Jandy, speciální testy na ramenní kloub- testy na AC skloubení, testy rotátorové manžety a impingement syndromu), na základě, kterého jsem navrhla krátkodobý rehabilitační plán. Fyzioterapeutická intervence trvala 3 měsíce v rozsahu dvakrát deset návštěv. K terapii jsem použila měkké a myofasciální techniky, presuru, Brüggerův koncept, cvičení v uzavřených kinematických řetězcích, vzpěrná cvičení dle R. Brunkow, ACT?, PNF). Součástí práce je popis terapie a výstupní kineziologické vyšetření s navržením dlouhodobého fyzioterapeutického plánu. Výsledky: během terapie došlo ke zlepšení rozsahu pohybu. Pohyb do flexe byl při vstupním vyšetření 50°, na konci terapie 175°,abdukce se zlepšila z 35° na 170°. Rozsah pohybu do vnitřní rotace nebylo při vstupním vyšetření možné vyšetřit, výsledná hodnota na konci terapie byla 85°. Pohyb do zevní rotace byl v počátku terapie operatérem zakázán. Po zhojení byla možná zátěž bez omezení, dosažený rozsah byl v závěru terapie 85°.Došlo ke snížení napětí měkkých tkání v okolí ramenního kloubu, zlepšení souhybu horní končetiny při chůzi a její zapojení při práci. The aim of my work is to contribute to solving physical therapy treatment impingement syndrome after arthroscopy shoulder surgery. Methodology: this is processed a case report of a male patient 53 year - old with diagnosis impingement syndrome after artroskopy treatment of rupture m. subscapularis, partial rupture caput longum m. biceps brachii, and rupture surfance sheet m. infraspinatus to the upper right limb. First I took a medical history, then an initial kineziological analysis (anthropometry, goniometry, the muscle test in Janda, static and dynamic tests, movement stereotypes in Janda, special tests on the shoulder joint- tests AC joint, rotator cuff, impingement syndrome). I made, on the basis of that I proposed a short-term rehabilitation plan. Fyzioterapeutic intervention lasted 3 months in the twice in then visits. I used to the therapy soft and myofascial techniques, presura, Brügger concept, excercise in closed kinematic chains, excecise in R. Brunkow, ACT?, PNF. Also included is description of the therapy and output kineziological analysis with desing long-term of fixed fyzioterapeutic plan. Results: during therapy come to improve range of motion. Movement to flexoin was in kineziological analisis 50°, at the end the therapy 175°, the abduction improved from 35° to 175°.ROM internal rotation was not possible to examinee at the entry examination, the value at the end of therapy was 85°. Movement external rotation was in the beginning therapy forbid, after healing was the burder without restriction, reach ROM was at the conclusion of therapy 85°.
