Oponent Alena Charvátová Abstrakt Abstrakt Úvod Tato bakalářská práce je seznámení čtenáře s problematikou dětské mozkové obrny a následné terapie pacienta s tímto onemocněním. Cíl práce Cílem práce je přispět k řešení problematiky metod fyzioterapie, postupů, konceptů a vypracování kazuistiky u pacientky s dětskou mozkovou obrnou. Metodika Pacientka docházela do ambulance po dobu 3 týdnů. Absolvovala 12 terapií, kdy všechny terapie trvaly v rozmezí 90-100 min. Během první terapie bylo provedeno vyšetření (anamnéza, aspekce, palpace, goniometrie, antropometrie, vyšetření spasticity, vyšetření přirozeného pohybu, vyšetření samostatné chůze, neurologické vyšetření) a na jeho základě byl vypracován cíl a krátkodobý plán fyzioterapie. Pacientka absolvovala 10 cvičebních jednotek, jejichž náplní bylo využití fyzioterapeutických metod a konceptů (mobilizace, protažení spastických svalů, pasivní pohyby, techniky měkkých tkání, DNS, využití nestabilních ploch, překážkových drah, nácvik kvadrupedální lokomoce ve vertikále, cvičení s pomůckami, cvičení v uzavřeném kinematickém řetězci, Motomed, chodící pás a jiné). Při poslední terapii bylo provedeno výstupní kineziologické vyšetření, které bylo podkladem pro zhodnocení terapie. Výsledky Výsledek rozdílu vstupního a výstupního měření: je patrné zvětšení rozsahu pohybu v hlezenních kloubech do dorzální flexe o 5 stupňů, zvětšení rozsahu do addukce u LDK a do abdukce u PDK u obou pohybů o 10stupňů. Při chůzi s berlemi pacientka již nezadrhává o předonoží a ve stoji již došlápne na celou plosku bez nutnosti slovní korekce. Závěr Dle výsledku je možno zhodnotit, že využití daných fyzioterapeutických postupů, mělo pozitivní vliv na stav pacientky. Summary Introduction This bachelor thesis is to introduce the reader to the issue of cerebral palsy and the subsequent therapy of patients with this disease. The aim of the thesis The aim of the thesis is to show the possibilities of using physiotherapy in pediatric patients with cerebral palsy. Methodology The patient went to the outpatient clinic for 3 weeks when she had 12 therapies, all of which lasted between 90-100 min. During the first therapy an examination (history, examination, palpation, goniometry, anthropometry, examination of spasticity, examination of natural movement, examination of independent gait, neurological examination) was carried out and on the basis of this examination a goal and a short-term plan of physiotherapy were developed. The patient underwent 10 exercise units involving the use of physiotherapy methods and concepts (mobilization, spastic muscle stretching, passive movements, soft tissue techniques, DNS, use of unstable surfaces, obstacle courses, quadrupedal locomotion training in the vertical, exercises with aids, closed kinematic chain exercises, Motomed, walking treadmill, etc.). During the last therapy, an exit kinesiological examination was performed, which was the basis for the evaluation of the therapy. Results As a result of the difference between the input and output measurements, there is an increase in the range of motion in the ankle joints into dorsiflexion by 5 degrees,, and an increase in the range into adduction for left lower extremity and into abduction for right lower extremity for both movements by 10 degrees. When walking with crutches, the patient no longer stumbles forefoot and in standing she can already step on the entire footplate without the need for verbal correction. Conclusion The results of the difference between the input and output measurements show some improvement in movement stereotypes.
