Existuje mnoho typů zlomenin, jako je například Collesova, Smithova nebo Rollandova, kde většina z nich vzniká pádem na nataženou horní končetinu. Řešení může být operativní či konzervativní. Avšak nedílnou součástí celého procesu je fyzioterapie, která je nezbytná pro návrat do aktivit běžného života a soběstačnosti. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s frakturou v oblasti distální části horní končetiny. Metodika: Praktická část obsahuje kazuistiku pacienta ve věku 53 let, který utrpěl frakturu distální části radia a ulny, která byla řešena operativně. Byl proveden vstupní kineziologický rozbor, na jehož základě byl stanoven krátkodobý fyzioterapeutický plán. Dále proběhlo deset terapií v období od 20. 9. 2023 do 15. 11. 2023. Během nich bylo využito míčkové facilitace, technik měkkých tkání, mobilizací, pasivních a aktivních pohybů, flossingu a mnoho dalších terapeutických pomůcek. Při poslední terapii byl proveden výstupní kineziologický rozbor, na jehož základě byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Při výstupním kineziologickým rozboru je patrné, že došlo ke zlepšení rozsahu pohybu do všech směrů, avšak největší zlepšení bylo pozorováno v metakarpofalangeálním kloubu palce o 50 stupňů při aktivním pohybu a v zápěstí do dorzální flexe o 35 stupňů v aktivním provedení, nepodařilo se nám však dosáhnout plného rozsahu pohybu. Zvětšení svalové síly vrostlo o jeden až dva stupně. Pacient nyní provede terminální, subterminální a palmární úchop, se kterými měl z počátku terapie veliký problém. Závěr: Výsledky, kterých bylo dosaženo pomocí fyzioterapeutických konceptů a metodik, které byly vhodně zvolené, ukazují, že přispěly ke zlepšení pacientova zdravotního stavu.. -- Abstrakt: Introduction: This bachelor thesis focuses on the diagnosis of fractures in the distal region of the radius and ulna. There are many types of fractures, such as Colles, Smith, or Barton fractures, most of which occur due to a fall on an outstretched upper limb. The approach to treatment can be either surgical or conservative. However, an integral part of the entire process is physiotherapy, which is essential for the return to the activities of daily living and independence. Aim: The aim of this bachelor thesis is to contribute to solving the issues of physiotherapy in patients with fractures in the distal part of the upper extremity. Methodology: The practical part includes a case study of a 53 years old patient who suffered a fracture of the distal radius and ulna, which was treated surgically. An initial kinesiological analysis was conducted, on the basis of which a short-term physiotherapeutic plan was established. Subsequently, ten therapy sessions were conducted from September 20, 2023, to November 15, 2023. These sessions utilized ball facilitation, soft tissue techniques, mobilizations, passive and active movements, flossing, and many other therapeutic tools. At the last therapy session, a final kinesiological analysis was performed, on the basis of which a long-term physiotherapeutic plan was established. Results: The final kinesiological analysis showed improvement in the range of motion in all directions; however, the most significant improvement was observed in the metacarpophalangeal joint of the thumb by 50 degrees in active movement and in the wrist in dorsal flexion by 35 degrees in active execution, although full range of motion was not achieved. Muscle strength increased by one to two grades. The patient can now perform terminal, subterminal, and palmar grips, with which they had significant difficulty at the beginning of therapy. Conclusion: The results achieved through the appropriately chosen physiotherapeutic concepts and methodologies indicate that they have contributed to the improvement of the patient's health condition.
