Komplex ošetřovatelských intervencí u pacienta s implantovaným kardiostimulátorem / Magdalena Nezbedová. -- 2016. -- 92 listů, 4 nečíslované listy příloh : grafy. -- Vedoucí práce Ivana Lamková. -- Abstrakt: V teoretické části se okrajově zabývám anatomií a fyziologií srdce, elektrickou aktivitou a vodivým systémem srdce. Dále je popsáno dělení arytmií, patogeneze, klinický obraz a diagnostika arytmií. Pacientům je popsán přístroj, průběh implantace, příprava na implantaci a průběh hospitalizace. Především se práce zabývá realizací ošetřovatelské péče po implantaci kardiostimulátoru. Pacienti po implantaci musí po dobu 24 hodin ležet v poloze na zádech. Na ráně musí být přiložený sáček s pískem z důvodu komprese rány. Pravidelně kontrolujeme fyziologické funkce, diurézu, stav pacienta a prosakování krytí. Podávají se tři dávky antibiotik v rámci profylaxe. Do dvou hodin po výkonu musí pacient podstoupit rentgen plic a srdce, aby se vyloučil pneumotorax.V závěru teoretické části je uvedena rešerše zkoumané problematiky. V empirické části je práce věnována specifikům ošetřovatelské péče bezprostředně po implantaci. Dále jsou podrobně rozepsány intervence sestry před a po implantaci trvalého kardiostimulátoru. Byla použita metoda,,case study". Bylo vybráno jedenáct respondentů, kteří byli indikováni k implantaci kardiostimulátoru. V závěru mé práce je uveden návrh ošetřovatelského standardu péče o pacienta po implantaci kardiostimulátoru.. -- Abstrakt: The theoretical part is marginally concerned with the anatomy and fysiology of the heart, its electrical activity and conducting system. It classifies arrhytmia as well as its pathogenesis, clinical picture and diagnostics. Patients are presented with the pacemaker, the course of implantation, preparation for implantation and hospitalization. The pacemaker is described. Above all, the work deals with the implementation of nursing care after pacemaker implantation. After the implantation patiens must lie on their back for 24 hours. The wound must be compressed by a bag of sand. Vital signs, diuresis and any signs of leakage are checked on regular basis.Three doses of antibiotic are administred to prevent infection. The patient must undergoan X-ray of the lungs and heart to rule out pneumotorax, within two hours after surgery. The research of this issueis discussed in my conclusion. The empirical part of the work is devoted to the specifics of the nursing care immediately after the implantation. Detailed nurse´s interventions (before and after the implantation of a permanent pacemaker) are described. ,,Case study "method was used. Eleven patiens ( selected for the implantation) were observed. At the end of my work I propose a draft of nursing care standards for the patiens after the pacemaker implantation.
